Create a Participant account if you have a rare disease*, condition, and/or diagnosis and will be answering questions about yourself.
Create a Caregiver account if you are a caregiver, guardian, or legally authorized representative that is helping care for an individual with a rare disease* and you have access to their medical records. If you have a rare disease yourself and also provide care for someone else with a rare disease, create a Caregiver account. If you are a grieving family member, create a Caregiver account.
* The term "rare disease" will be used on this platform as an umbrella term referring to a rare disease, condition, diagnosis, and/or disorder.
